David Iley Cook's Notes...
Iley is the informant on Jefferson Davis Cook's death certificate and I believe buried next to him at Bethlehem Cemetery in Wickliffe, Kentucky. Iley can be found on the 1900 census for Carlisle County, Kentucky with his mother and 2 brothers and on the 1910 census in Ballard County, Kentucky as Iley L Cook with his wife Marvis. Bethlehem Cemetery had a fire and the records were burned but there are what appear to be 2 grave without stones next to the family graves for Great Uncle Iley, his wife and son. I suspect they are the graves of Mary Emma Roach Cook and Jefferson Davis Cook. I could not locate a death certificate for Mary but Jefferson's does state that he is buried at Bethleham.
